This Inspection/ Audit was completed for two purposes: 1) Improve the Safety Culture (25 OSHA Recordable Accidents since beginning of year), 2) Reinstate the canceled insurance. Duties included, but were not limited to: Initially, all documentation was reviewed, documented and updated. This followed an OSHA inspection resulting from an incident and citations. The documentation includes: Medical records - Quantitative / Qualitative medical examination of workers whose jobs required them to be in an usually hazardous environment. Reviewed the history, education, certification, etc. of physician(s) performing medical examinations, Medical surveillance, Operational and Performance Respiratory Fit-Test, Medical qualifications required to perform work in SCBA PPE. Training records, OSHA 300 Recordkeeping. Reviewed and advised concerning sub-contractor safety and health preparations as cross-matched to the Company's Safety Policies and Procedures. Set up procedures to evaluate sub-contractor operations. This includes both the "one-man" sub-contractor through multi-employee sub-contractor. Evaluated the facility monitoring and security procedures. This evaluation resulted in the establishment of new facility monitoring and security procedures. Established facility monitoring and security procedures. Worked with company management to establish a time and motion study to improve the workflow while utilizing this large scale operation. Observed improper heavy lift (in excess of 100,000 pounds) procedures. Worked with engineering to establish safe-work procedures when a heavy lift is required. Processed the safety inspection/audit involving 29 CFR 1910, 1918 and 1926. Safety inspection/audit included, but not limited to, manufacturing, service and administration. Set up sub-contractor policies and procedures, especially welders. At any one time there were more than 2,000 sub-contractor welders, none of which fulfilled the consideration of safe-work practices until contractor policies and procedures were implemented.
